Those of you that were online at the time will know what this is...others amy be confused. ANYWHO i am doing a first aid course, and after 6 hours work, i had to come home and do exam style questions SO i thought i'd just post them here so you can all feel my great pain xD
one : You arrive at a RTC there is an unconcious casulty at the side of the road & some one else sitting nearby shouting for help. Please explain how you would treat the casutly or casulties.
my answer : 1.) Call 999/112 immediately. The person that is shouting out is obviously in some sort of ‘okay’ state, as she is shouting (therefore; breathing) and doesn’t seem to have any serious injuries. Send the conscious person to stop oncoming traffic (signalling at the side of the road) and focus on the unconscious patient. They are not lying in an awkward position, however, it still could be dangerous to move their head ( they could have spinal or neck injuries) Check for any other dangers, if seeing none kneel beside the unconscious person and check for any blockages to the airways, if there is none, check breathing. If the patient is breathing, reassure and talk to them. (only putting them in the recovery position if it is absolutely necessary) If they are not breathing, begin CPR immediately. Using 30 compressions, then two breaths and repeating this procedure until the patient starts breathing or the ambulance arrives.
If the person comes conscious before the ambulance arrives, reassure them, tell them that an ambulance is on its way and everything will be fine, explain how the person that was with them is just around the corner warning traffic and that they are fine. Keep them still, tell them it could be unwise to move until professional help is here.
two : You are on a hack with a group of riders, a tree catches one rider. The branch hits the rider but doesn't dismount them however, there is suspected limb fractures. Please explain how you would manage to treat the rider.
my answer : 2.) The rider that was caught by the branch appears to have a fractured wrist and are remaining calm. Keeping a calm attitude, move the ride away and carry on as normal. One member of staff is to stay with the casualty, whilst the other members continue with the ongoing ride. Carefully dismount the rider, taking hold off the horse and reassuring the rider. Contact the yard immediately and support the wrist, making sure to monitor for shock. Keep talking to them to reassure them and keep their mind off the pain. Once back at the yard, sit the patient down and arrange for transport to the nearest hospital.
three A : Discribe what you would do if someone at the yard approched you & appeared to be dazed, not sure of their location, looked hot and flushed and continued to repeat themselves. They have mud on their riding hat. When asked if they had fallen they could not remember.
my answer : 3a.) It is obvious that this rider has fallen (mud on hat) and there could possibly be damage to the neck, back or head as they are dazed. Keep a calm attitude and reassure the person and trying to get them to concentrate on you, rather than gazing into nothing. Immediately dial 999 / 112 and request an ambulance, stating that is a possible neck/spinal/head injury. Try not to move the person too much, but attempt to support them against something (car bonnet/ table) making sure the neck and head are straight and secured. Monitor their breathing and check for any signs and symptoms of shock. Do not remove the riding hat, and if they attempt to, gently negotiate with them not too. Do not give them any food or drink, even if it is requested this may lead to vomiting or problems in an operation if the injury turns more serious. Continue to talk to them, trying to start a conversation and keep them concentrated.
three B : A casulty is aggressive and combative refuses treatment and tries to walk away. List the important points to remember when dealing with this person.
my answer : 3b.) It is important to stay calm in a situation like this. You will get no where if you shout back at this casualty. Reassure this person, walk with them, but not too close, it is important that this person doesn’t wander off alone (as they may cause an accident either to themselves or others) however, it is vital to think of your safety first. Try to negotiate with this person, asking if they would return to the yard to talk sensibly. Ask if there are family members / friends nearby, ( as they might listen to them more) If family / friends arrive and the person doesn’t improve, ask them if they are on any medical treatment. If they still refuse to take it, it would be wise to contact emergency services in case this treatment / medicine is needed.
four : While out on a ride, you see a horse galloping without its rider and not far away ,you see a rider lying on the floor in great pain and unable to move but demanding their hat and body protector to be removed. Please state the most important things to remember in this situation.
my answer : 4.) It is important to remove any dangers (any horses etc) as the air ambulance will be called (could spook the horses, and cause further injuries) After ringing for the air ambulance, ask the casualty where the body is hurting, trying to make a calm conversation with them. As they are in great pain and unable to move, it is most likely a spinal related injury. Kneel by their head and support head with your knees, making sure the body is as straight as possible. Explain why it would be very dangerous to remove the riding hat and body protector, as the hat will be compressing any bleeding and the body protector will be acting as a support and spreading out the pain. Monitor their breathing and check for any signs of shock. Attempt to keep the patient calm, ask what has happened and tell them that help is on the way.
five A : A child in your professional care gets very cold and wet while riding. please say how you would deal with this.
my answer : 5a.) Dismount the rider and get them moving (star jumps, jogging on the spot etc) do not cuddle or rub the child in an attempt to keep them warm. Take off first layer (only) of clothing and give a warm dry jacket. Once back at the yard, find the child’s parent(s) give the child a warm drink and a warm, dry jacket.
five B : Upon examination, you find minor incised wounds to the left palm and round bruising marks to the inside of the limbs, list your actions.
my answer : 5b) Contact the yard manager immediately and do nothing on your own, always have someone with you. Write everything down that you see in detail, including dates, times and signatures. If the child is a local rider and returns weekly / monthly monitor behaviour or attitude, look at physical appearance and if the child worsens, consider contacting social services.
